REVENUE MEMORANDUM CIRCULAR NO. 127-2019 issued on November 27, 2019 notifies the loss of ten (10) sets of unused BIR Form No. 0423 – Apprehension Slip with Serial Numbers APS200300043661 to APS200300043670.

The abovementioned forms, which were reported as lost by Revenue Officer Jenifer O. Dalioan of Revenue District Office No. 6, have been cancelled and all official transactions involving the use thereof are considered as invalid.
