The abovementioned forms, which were reported as lost by Senior Administrative Assistant Celedonia P. Tenorio of Revenue Region No. 9A-CaBaMiRo, have been cancelled (including the entire eCP set bearing the said Serial Number of the missing original copy) and all official transactions involving the use thereof are considered as invalid.
