REVENUE MEMORANDUM CIRCULAR NO. 114-2019 issued on October 22, 2019 notifies the loss of three (3) pads of unused/unissued Accountable Form No. 51 – Official Receipt with Serial Numbers ACF201100702501 to ACF201100702650.

Said forms, which were reported as lost by Administrative Section Chief Cory Eliesa C. Quivis of Revenue Distrtict Office No. 76, have been cancelled and all official transactions involving the use thereof are considered as invalid.
