REVENUE MEMORANDUM CIRCULAR NO. 11-2017 issued on February 6, 2017 notifies the loss of one (1) pad of unused BIR Form No. 0535 – Taxpayer Information Sheet bearing serial nos. TIS201400254001 to TIS201400254050.

     Said forms, which were reported as lost by Ms. Glenda C. Sison of Revenue District Office No. 99, have been cancelled and all official transactions involving the use thereof are considered invalid.
