REVENUE MEMORANDUM CIRCULAR NO. 101-2018 issued on December 11, 2018 notifies the loss of eight (8) sets of unused BIR Form No. 0535 – Taxpayer Information Sheet with Serial Numbers TIS201400239393 to TIS201400239400.

         Said forms, which were reported as lost by Mr. Gerardo B. Dizon of Revenue District Office No. 5, Caloocan City, have consequently been cancelled and all official transactions involving the use thereof are considered invalid.
