REVENUE MEMORANDUM CIRCULAR NO. 1-2006 issued on January 10, 2006 notifies the loss of the original copy from one (1) set of used BIR Form No. 17.58 – Authority to Cancel Assessment with Serial No. ACA97-000014262.

     Said copy, which was reported lost by Ms. Mila E. Navarro of Revenue Region No. 7, Quezon City, has been cancelled and all official transactions involving the use thereof is considered invalid.
