REVENUE MEMORANDUM CIRCULAR NO. 15-2026 issued on March 9, 2026 notifies all internal revenue officials, employees and others concerned about the loss of original and duplicate copies of used/filled-up but unissued BIR Form No. 1966 – Electronic Letter of Authority with Serial Number eLA202000029581.

Said form, which was reported as lost by Revenue Officer Addi B. Collamar of Revenue District Office No. 87, has been cancelled and all official transactions involving the use thereof are considered as invalid.
