REVENUE MEMORANDUM CIRCULAR NO. 69-2025 issued on July 7, 2025 notifies the loss of one (1) pad of unused/unissued BIR Form No. 0535 – Taxpayer Information Sheet with Serial Numbers TIS202300179001 to TIS202300179050.

Said forms, which were reported as lost by Administrative Officer IV Julicer K. Oseña of Revenue District Office No. 58, have been cancelled and all official transactions involving the use thereof are considered as invalid.
