REVENUE MEMORANDUM CIRCULAR NO. 36-2025 issued on April 10, 2025 notifies the loss of one (1) set of used/filled-up but unissued BIR Form No. 0422 – Mission Order with Serial Number MSO201700024118.

Said form, which was reported as lost by Revenue Officer Marie-Geraldine C. Bucag of Excise LT Field Operations Division, has been cancelled and all official transactions involving the use thereof are considered as invalid.
