REVENUE MEMORANDUM CIRCULAR NO. 67-2017 issued on August 10, 2017 notifies the loss of six (6) unused sets of BIR Form No. 0423-Apprehension Slip bearing Serial Nos. APS200100007578 – APS200100007583.

     Said forms, which were reported as lost by Mr. Sixto C. Dy Jr., former Chief of the National Investigation Division, have been cancelled and all official transactions involving the use thereof are considered invalid.
