REVENUE MEMORANDUM CIRCULAR NO. 70-2009 issued on December 14, 2009 notifies the loss of one (1) original copy of Tax Credit Certificate (TCC) with Serial Number R-018309 issued to Diretso Design Furnitures, Inc.
     Said form was reported lost by Mr. Teodoro G. Galicia of RDO No. 49, North Makati. Consequently, the entire set bearing the said TCC Number has been cancelled and all official transactions involving the use thereof effective July 28, 2009 are considered invalid.
