REVENUE MEMORANDUM CIRCULAR NO. 23-2005 issued on June 20, 2005 notifies the loss of five (5) sets of unissued BIR Form No. 0423 – Apprehension Slip (APS) with Serial Numbers APS200300026583 to APS200300026587.
         Said forms, which were reported lost by Ms. Dirce M. Diaz of RR 7, Quezon City, have been cancelled and all official transactions involving the use thereof are considered invalid.
