REVENUE MEMORANDUM CIRCULAR NO. 8-2007 issued on February 2, 2007 notifies the loss of 1 set and the duplicate and triplicate copies from another 1 set of unused BIR Form No. 0423 – Apprehension Slip with the following Serial Numbers:

                                                 APS200200022255 – one (1) set

                                                 APS200200005009 – duplicate and triplicate copy

             Said forms, which were reported lost by Ms. Evelyn C. Ignacio of Revenue Region No. 5, Valenzuela City, have been cancelled and all official transactions involving the use thereof are considered invalid.
