REVENUE MEMORANDUM CIRCULAR NO. 7-2004 issued on February 6, 2004 notifies the loss of one (1) quadruplicate copy of an unused Revenue Official Receipt BIR Form No. 2524) with Serial No. ROR200301341101.

    Said form, which was reported lost by Ms. Sonia M. Atip of BIR-Administrative Division, Revenue Region No. 14, Candahug Palo, Leyte, has been cancelled and all official transactions involving the use thereof are considered invalid.
