REVENUE MEMORANDUM CIRCULAR NO. 41-2006 issued on July 13, 2006 notifies the loss of one (1) set of unused BIR Form No. 1954-Certificate Authorizing Registration with Serial No. CAR200500265354.

     Said form, which was reported lost by Ms. Arlyn H. Imbo of Revenue District Office No. 55, San Pablo City, has consequently been cancelled. All official transactions involving the use of said form are considered invalid.
