REVENUE MEMORANDUM CIRCULAR NO. 38-2006 issued on July 5, 2006 notifies the loss of 2 triplicate copies and 2 sets of cancelled BIR Form No. 1965 – Letter of Authority (LOA) with the following serial numbers:

     Said forms, which were reported lost by Ms. Ma. Yvonne D. Jabas of Large Taxpayers District Office No. 123, Cebu City, have been cancelled and all official transactions involving the use thereof are considered invalid.
