REVENUE MEMORANDUM CIRCULAR NO. 32-2009 issued on June 23, 2009 notifies the loss of one set of unused BIR Form No. 0421 – Tax Verification Notice (TVN) with Serial No. TVN200200204282.
     Said form was reported lost by Mr. Kenneth A. Martin of Revenue District Office No. 89, Revenue Region No. 14, Government Center, Candahug, Palo Leyte. Consequently, the entire TVN set bearing the said serial number has been cancelled and all official transactions involving the use thereof are considered invalid.
