REVENUE MEMORANDUM CIRCULAR NO. 11-2006 issued on February 14, 2006 notifies the loss of one (1) pad of unused BIR Form No. AF51-Accountable Form 51 with Serial Nos. ACF200400728001 to ACF200400728050.

     Said forms, which were reported lost by Ms. Gloria H. Mercado of Revenue Region No. 9, San Pablo City, have been cancelled and all official transactions involving the use thereof are considered invalid.
